We added the name, email, phone, date requested, date needed, purpose, and amount field for the first part of the form.

After saving the form go to the Settings & Integrations section from the top.

In the Form Settings, you can change Confirmation Settings, Form Layout, Scheduling & Restrictions, Advanced Form Validation, Survey Result, Compliance Settings, and Other.

We’ll set up the form Confirmation Message to appear on the same page and after the submission form will hide automatically. You want otherwise you can do that too. Like show the confirmation message on another page or after the submission form will reset.
