About this template...
This graphic design quiz form helps design instructors, bootcamps, creative agencies, and hiring managers check knowledge of core design principles. It covers color theory, print and screen color models, typography, file formats, image types, and layout, giving a clear picture of someone’s foundational design skills.
The form collects first name, last name, and email for identification and result tracking. Ten multiple-choice questions cover primary colors, CMYK, kerning, transparent file formats, vector versus raster images, complementary colors, serif typefaces, white space, DPI, and visual hierarchy.
Once submitted, the form scores responses automatically and can show results in the confirmation message or by email. You can add software-specific questions, focus on branding or UI design, or use it as a quick screening step for junior design roles.
