About this template...
This probability quiz form helps math teachers, tutors, and test-prep providers measure how well learners understand chance and likelihood. It covers basic probability, complementary events, combined outcomes, and expected results, using familiar examples like coins, dice, cards, and marbles.
The form collects first name, last name, and email for identification and result tracking. Ten multiple-choice questions cover coin tosses, single die rolls, certain and impossible events, picking from a bag, even-number outcomes, the chance an event won’t happen, flipping two coins, drawing from a deck of cards, and expected frequency over repeated rolls.
Once submitted, the form scores responses automatically and can show results in the confirmation message or by email. You can change the scenarios, use decimals or percentages instead of fractions, or raise the difficulty for older students.
