About this template...
This letter of recommendation request form helps students, alumni, and employees ask teachers, professors, advisors, and managers for a letter without a long email chain. Recommenders get the deadline, purpose, and talking points in one organized request, so writing the letter takes less back-and-forth.
The form starts with the requester’s contact details and current status, then asks how they know the recommender, including the class, program, or role they shared and how long they have worked together. It also collects the recommender’s name, email, and department so the request reaches the right person.
Requesters then explain what the letter is for, the institution and program, the deadline, and how the letter should be submitted, along with their waiver status. They can choose the qualities they want highlighted, share key accomplishments and goals, upload a resume or CV, and acknowledge the preparation time before sending the request.
