This training application form helps course providers, bootcamps, and corporate training teams collect complete applications for any program they run. Applicants pick a course, batch, and delivery mode, then move through personal and contact details before describing their background and goals, so your team gets everything it needs to plan cohorts and confirm seats.
The course selection section covers which program the applicant wants, their preferred schedule, and whether they’d rather learn online, on campus, or a mix of both. Personal and contact fields follow, with gender offering a “prefer not to say” option so applicants aren’t forced to disclose more than they want.
The background section asks about current status and prior experience and only asks applicants to describe their past projects when they’ve said they have intermediate or advanced experience. A closing section captures what applicants hope to get out of the training and how they found your program, plus a quick acknowledgment that submitting the form registers their interest.
