About this template...
A performance improvement plan works only when the expectations, timeline, and support structure are documented clearly from the start. When PIPs are handled through informal conversations or loosely written emails, both the employee and the manager are left without a reliable reference point for what was agreed upon. A Performance Improvement Plan Form Template structures that process into a single, consistent record that both parties can refer to throughout the improvement period.
This form uses Conditional Logic tied to a Dropdown field to adapt the content based on the type of performance issue being addressed. Whether the concern is attendance, quality of work, behavioral conduct, or missed targets, the form shows only the fields and sections relevant to that specific category. Each variation is wrapped in a One Column Container that appears or disappears based on the dropdown selection, keeping the form focused rather than presenting every possible field at once.
Section Breaks divide the form into distinct stages: employee details, performance concern, improvement objectives, support to be provided, and review timeline. Name, email, and Time and Date fields establish who the plan applies to, who is filing it, and when the improvement period begins and ends. Text Area fields give managers space to describe the specific performance gap, outline measurable goals the employee is expected to meet, detail the resources or support the organization will provide, and define the consequences if the plan is not fulfilled.
The result is a performance improvement plan template that is thorough enough to protect both the employee and the organization, while remaining practical enough that managers will actually complete it. Every submission creates a timestamped, structured PIP record that HR can store, track, and reference throughout the review period and beyond.
